CHS Game Tickets - Now Online Only
Carlsbad High School has moved to online ticketing for all athletic events.
Tickets are no longer sold at the gate.
To attend, purchase your tickets online and redeem them on your device when you arrive.
Use the GoFan App
We recommend downloading the GoFan app to easily store and access your tickets.
You can also transfer tickets to friends or family via text or email.
Without the app, you’ll need to retrieve tickets from your email.
Staff Admission
CMSD staff and their immediate family members can attend games for free
by showing a valid district ID at the gate.
IDs may not be transferred to others.

NFHS Concussion Course
Free CDC-backed course teaching students how to recognize, respond to, and recover from concussions. Each student must register individually.

Student Sports Physical Packet
Includes history, physical exam, and medical eligibility forms required for athletic participation. Each student must submit their own signed form.

Student Accident Claim Forms & Brochures
If your child is injured during a school event or sports activity, you can use these forms to file a claim. Forms and brochures explaining the coverage are available in English and Spanish.

Extracurricular Forms Portal
Complete required forms for sports and activities online, including consent, conduct, and eligibility. Sign digitally, no printing needed.
Student Accident & Injury Insurance

Our schools provide accident and catastrophic injury insurance for all students who take part in school-sponsored activities, including sports. This coverage helps make sure your child can get the care they need if they’re hurt during a school activity.
What’s Covered
This insurance covers:

All school-sponsored, school-supervised activities
-
All school sports, including practices, games, and travel to events

Injuries that happen during other school events
How It Works

If your child has their own health insurance: This plan helps pay costs your main insurance doesn’t cover, like co-pays and deductibles.

If your child doesn’t have health insurance: This plan will be the main accident coverage.

You can visit any licensed doctor or health care provider.

We can reimburse you if you’ve already paid for a covered service, or pay the doctor or hospital directly if they bill us.
How to File a Claim

School fills out Part A

A school staff member who saw the accident will fill out and sign Part A of the claim form.

The form will be given to you.

You fill out Part B
-
Follow the instructions on the form.

Include the documents and signatures needed.

Send the form to Myers-Stevens & Toohey (MST) using the contact information on the form.

Keep a copy of the form for your records.
